What is Gestational Weight Gain?

Gestational weight gain (GWG) is the total weight gained between conception and delivery. It is not simply fat - it includes the weight of the fetus (~3.3 kg at term), placenta (~0.7 kg), amniotic fluid (~0.8 kg), expanded blood volume (~1.5 kg), uterine and breast tissue growth (~1.4 kg combined), extracellular fluid (~1.5 kg), and maternal fat stores (~3-4 kg built as an energy buffer for breastfeeding.

The Institute of Medicine (IOM) published evidence-based GWG guidelines in 2009, updated from the 1990 version to reflect modern obesity prevalence. These guidelines set total gain targets and per-week rates by pre-pregnancy BMI category, and are endorsed by ACOG, the CDC, and most obstetric bodies worldwide.

GWG outside the recommended range - either too much or too little - is associated with distinct adverse outcomes. Common pitfalls

The most common mistake is applying the normal-weight target (11.5-16 kg) to women with overweight or obesity, leading to excess gain. A second pitfall is expecting first-trimester gain to match the per-week rate: the weekly rate applies only from the second trimester; minimal or even negative first-trimester gain due to nausea does not need to be "made up."
